Chief complaints: 

Poor vision right eye 

‎‏past medical ophtmological history:

Traumatic injury right eye when he was a kid , got cataract and done cataract surgery right eye.

On examination:

‎‏unaided visual acuity:

OD - HM

OS 6/12  

Subjective refraction:

OD: no improvement

OS : 0.00/ 0.75 x 80 

IOP:

OD: 17.3.     OS: 15

_Slit lamp examination _

‎‏*conjunctiva * : unremarkable   OU

‎‏cornea 

‎‏OD .clear       OS clear

‎‏*pupil  *

‎‏OD.      Irregular synechiae with IOL

‎‏OS.      PERRLA

‎‏lens 

‎‏OD: PCIOL subluxated IOL in front of iris + PCO

‎‏OS clear

‎‏Diagnosis 

‎‏Subluxated IOL + PCO OD
